摘要
目的:探讨外周静脉穿刺中心静脉置管治疗血液病中途拔管的危险因素。方法:采用病例对照研究策略,将我科应用PICC置管治疗的65例血液病患者随机分为病例组25例和对照组40例,并对两组可能影响导管留置因素予以观察记录,其数据在单因素分析的基础上,又作Logistic多元回归分析,以遴选危险因素。结果:病例组置管后局部渗血、静脉炎发生率分别为56%和44%,分别比对照组相应率44%和15%均高(P<0.05,OR>1,其95%可信区间内不包含1)。结论:置管后局部渗血和静脉炎为PICC留置治疗血液病中途拔管的危险因素,应强化防范措施。
Objective：To identify the potential risk factors that lead to discontinued clinical use of peripherally inserted central catheter （PICC） treatment in patients with hematological diseases. Methods：65 hemopathic patients with PICC treatment were divided into treatment group（n=25） and control group（n=40） at random. The potential factors related to discontinued use of PICC were analyzed with One-way ANOVA and Logistic multiple regression methods. Results： The incidences of local errhysis at insertion site and phlebitis in the PICC treatment group were higher than those in control group （56% vs 44%; 44% vs 15%,respectively,all P 0.05; OR1）. Conclusion： The preliminary results suggested that local errhysis at insertion site and phlebitis may be two main risk factors of prematurely extubation during PICC treatment in hemopathic patients.
