医院社区互动糖尿病患者护理模式的建立与运行

Development and Implementation of Hospital-community Interactive Nursing Mode for Patients with Diabetes

成立了院糖尿病护理小组、医院社区双向转诊中心、住院宣教社区访视小组、家居自我管理小组,实行糖尿病患者优先就诊、检查、住院,初步形成了社区与医院、专业护士与社区护士、护士与患者之间的互动,为糖尿病患者提供及时、便利、连续性、全程护理服务。该模式取得了预期的良好效果,其形式和内容得到了患者认可,660例糖尿病患者满意率为97.5%。

The interactive mode between community and hospital,specialist nurses and community nueses,as well as patients and nurses was formed to provide timely,convenient,continuous and whole-process nursing care for diabetic patientsnity,hospital-based education and community-based visit team,community-based self-management group as well as green channel of priority treatment,inspection and hospitalization for diabetic patients.The implementation of the interactive mode achieved good effect and was approved by the patients.The patieng satisfaction rate was97.5%.
