脑室腹腔分流术加伽马刀治疗颅内肿瘤合并梗阻性脑积水 被引量:2

Treatment of Intracranial Tumors Accompanied by Obstructive Hydrocephalus by Ventriculoperitoneal Shunt and Gamma Knife

摘要 目的回顾总结28例颅内肿瘤合并梗塞性脑积水先行脑室腹腔引流术后,再行伽马刀治疗的具体方法及疗效。方法对入院治疗患者先行脑室腹腔分流术缓解高颅压症状.通畅脑脊液循环后,再行颅内肿瘤的伽马刀治疗。结果行脑室腹腔分流术后,本组28例高颅压症状均明显缓解。术后3d复查头颅CT见脑室较术前有不同程度缩小,术后5~8d行超级伽马刀治疗。随访1个月~60个月,一般半年复查1次。肿瘤完全消失9例.11例肿瘤不同程度缩小,3例肿瘤无明显变化,5例复查2~3a后肿瘤消失再复发后再行伽马刀治疗。结论超级伽马刀加脑室腹腔分流术是颅内肿瘤并梗阻性脑积水的有效方法之一.也是首选方法. Objective Reviewed and summarized eflqcacy and specific methods of 28 eases of intracranial tumor accompanied by obstructive hydrocephalus,first by Ventriculoperitoneal shunt and then by Gamma Knife. Methods First ventriculoperitoneal shunt was used for patients to relieve symptoms of high intracranial pressure, to make eerehrospinal fluid circulation easily, the further the gamma knife treatment of intracranial tumors was used. Result 28 patients in this group were significantly ease symptoms of high intracranial pressure after ventriculoperitoneal shunt. The ventricle redduced than before surgery according to re.salt of head CT after surgery 3 day. The intraeranial tumors was treated with gamma knife after surgery 5-8 days. Follow-up from 1 month to 60 months, six month~ review one generally. Tumor completely disappeared in 9 eases, and the tumor of 11 cases reduced varying degrees , 3 eases with no obvious changes. The tumor of 5 cases disappeared after follow-up 2-3 years, and recurrence , then gamma knife treatment. Conclusion Gamma Knife and ventrieuloperitoneal shunt is one of effective method in the treatment of brain tumor and obstructive hydrocephalus, and the preferred method.
