摘要
目的探讨2型糖尿病患者瘦素水平与认知功能的关系。方法对85例2型糖尿病患者进行认知问卷,评定认知功能,并测定患者血清瘦素、胰岛素、血糖、血脂、促甲状腺激素(TSH)等水平。结果不同瘦素水平的2型糖尿病患者认知功能有统计学差异(P<0.05);瘦素与MMSE总分、MMSE即刻记忆1、MMSE执行能力、言语流畅性等呈明显负相关(P<0.05),与指图总耗时及图片误认数呈正相关(P<0.05);瘦素与性别、年龄、胰岛素抵抗指数(HOMA-IR)、TSH、体重指数(BMI)以及是否合并高血压、冠心病等也密切相关(P<0.05)。多元逐步回归分析显示,HOMA-IR、性别及BMI为影响瘦素水平最显著的因素。结论瘦素可加重2型糖尿病患者的认知功能损害;瘦素水平与HOMA-IR、性别和BMI等密切相关。
Objective To study the relationships of leptin and the cognitive function in the patients with type 2 diabetes mellitus(T2DM).Methods Questionnaires were used to evaluate the cognitive function of 85 T2DM patients.Serum leptin and insulin,blood glucose,blood lipid,thyroid stimulating hormone(TSH) were determined.The patients were divided on the basis of leptin≥5 ng/ml(group A) and 〈5 ng/ml(group B).The cognition and the factors correlated to leptin were analyzed between two groups.Results Serum leptin was significantly different between groups of A and B(P〈0.05).Serum level of leptin was negatively correlated to composition of picture of mini-mental state examination(MMSE),vision inference,the first auditory immediate memory,comparability,motor testing,and positively correlated to the right numbers of filling the lack of pictures the wrong numbers and the expended time of cancelling test(P〈0.05).Serum level of leptin was closely related to sex,age,homeostasis model assessment for insulin resistance (HOMA-IR),TSH,body mass index(BMI) and complicated with hypertension and coronary heart disease(CHD) (P〈0.01 or P〈0.05),of which HOMA-IR,sex and BMI were the most significant factors to effect serum leptin.Conclusion Leptin aggravates cognitive dysfunction in T2DM patients.The level of leptin in T2DM patients has a close correlation with HOMA-IR,sex and BMI.
