摘要 目的评估Mast Quadrant可扩张管通道微创手术系统局部"开窗"减压治疗腰椎椎管狭窄症的临床效果。方法 2008年2月~2009年6月本院52例腰椎椎管狭窄症患者随机分成2组,其中26例接受Mast Quadrant可扩张管通道微创手术系统辅助下椎管减压术,26例接受开放椎管减压术。结果微创组和开放组手术时间分别为40~110min和35~120min,平均76min和80min;2组比较差异无统计学意义,术中出血量分别为50~230mL和80~320mL,平均110mL和150mL,2组比较差异有统计学意义。术后平均随访7.2个月。末次随访时采用改良Macnab标准评价临床效果,微创组优良率为92.3%,开放组优良率为93.3%,两者比较差异无统计学意义。结论与传统开放手术相比,Mast Quadrant可扩张管通道微创手术系统辅助下局部"开窗"减压治疗腰椎椎管狭窄症可以获得相同的临床疗效,但具有手术时间短、术中出血量少、组织损伤轻和术后恢复快等优点。 Objective To evaluate the clinical outcomes of minimally invasive windowing decompression assisted by Mast Quadrant system in treatment of lumbar spinal stenosis.Methods From february 2008 to June 2009,a total of 52 patients with lumbar spinal stenosis were divided into 2 groups randomly.Twenty-six cases in 1 group underwent minimally invasive windowing decompression assisted by the Mast Quadrant system(Group micro-invasive)and the others underwent windowing decompression by regular open incisal opening(Group regular).Results In Group micro-invasive,operation time was 40-110 min(76 min in average),and blood loss was 50-230 mL(110 mL in average).In Group regular,operation time was 35-120 min(80 min in average),and blood loss was 80-320 mL(150 mL in average).All the patients were followed up and the mean period was 7.2 months.Clinical outcomes were determined using the modified MacNab criterion,the excellent and good rate was 92.3% in micro-invasive group and 93.3% the regular group.Conclusion Compared with regular opening way,minimally invasive windowing decompression assisted by Mast Quadrant system for the management of lumbar spinal stenosis has the similar clinical outcomes,but it can minimize blood loss and tissue trauma,shorten the operative and recovery time.
