摘要
目的:探讨超声检查时肾动脉舒张期反向血流在移植肾中的意义。方法:对632例移植肾共1960次Doppler频谱检查资料进行回顾性分析,并与临床及手术病理结果相比较,其中2例患者行超声造影。结果:22例移植肾患者表现为舒张期反向血流,其病因包括急性排斥反应(12例)、肾小管坏死(5例)、肾动脉血栓(1例)、肾静脉血栓(2例)和感染(2例)。舒张期反向血流频谱形态分为舒张早期(8例),舒张晚期(2例)和全舒张期(12例)。全舒张期血流频谱形态可再分为平台型(4例)、反M型(6例)和V型(2例)。结论:对舒张期反向血流连续观察有助于判断移植肾预后,但移植肾舒张期反向血流频谱形态不能区分其病因,超声造影有助于其病因诊断。
Objective:To study the significance of reversed diastolic flow (RDF) on Doppler ultrasonography in transplanted kidney. Methods:Ultrosonography materials in 1960 times of Doppler sonogram in 632 cases with renal transplantation were retrospectively analyzed including 2 underwent contrast enhanced ultrasonography. Correlation with surgery, histo-pathology and clinical outcomes were performed. Results:22 patients had transplantation of kidney showed RDF,the causes were acute rejection (n= 12),acute tubular necrosis (n=5),arterial thrombosis (n= 1),venous thrombosis (n=2) and infections (n=2). The RDF patterns included early diastole type (n=8),total diastole type (n: 12) and late diastole type (n= 2). Patterns of total diastole type of Doppler ultrasonography could be further subdivided into"plateau" subtype (n: 4) ,"inverted M" subtype (n= 6) and "V" subtype (n= 2). Conclusion: Consecutive study of the transplanted kidney with Doppler ultrasonography showing RDF may help to predict the prognosis of the patient, but the causes of RDF cannot be distinguished by their patterns. Contrast enhanced ultrasoography might provided helpful information for the diagnosis of the cause of RDF.
