肺外炎性肌纤维母细胞瘤的MRI表现与病理对照分析

MRI Features of Extrapulmonary Inflammatory Myofibroblastic Tumor with Pathological Correlation

目的 探讨肺外炎性肌纤维母细胞瘤(IMT)的MRI表现,以期提高对其认识及诊断水平。资料与方法回顾性分析12例经病理证实的肺外IMT患者的MRI表现,并与病理进行对照分析。结果 发病部位:软组织8例,骨骼3例,蝶窦1例;MRI主要表现:与肌肉信号相比,T1WI序列均表现以等信号为主,T2WI序列表现均以高信号为主,其内散在点状和(或)条索状低信号,增强扫描7例均匀强化,3例液化坏死区无强化,2例由边缘向中央强化。病理:10例以肌纤维母细胞、炎性细胞为主,2例以丰富的胶原纤维为背景。结论 肺外IMT的MRI表现具有一定的特异性,能为肿瘤的定性及定位诊断提供重要价值。

Objective To study the MRI appearances of extrapulmonary inflammatory myofibroblastic tumor（IMT）,in order to improve the diagnosis.Materials and Methods The MRI appearances of 12 cases with extrapulmonary IMT were reviewed and compared with pathomorphologic findings.Results In 12 cases of IMT,8 tumors located in soft tissue,3 located in bone and 1 located in sphenoidal sinus.On MR imaging,compared with the muscle signal intensity,the IMT lesions presented intermediate signal intensity on T1-weighted images,hyperintensity with spot or mottling slightly hypointensity on T2-weighted images.After contrast enhancement,7 cases showed homogeneous enhancement,3 cases showed heterogeneous enhancement,2 cases enhanced from the edge to the center.Pathological findings revealed characteristic myofibroblast and phlegmonosis cells in 10 cases,amount of extracellular collagen in 2 cases.Conclusion Extrapulmonary inflammatory myofibroblastic tumor has its features on MRI imagings.MRI imagings can provide helpful information for the clinical diagnosis of inflammatory myofibroblastic tumor.
