摘要
目的 评价16层螺旋CT血管成像(CTA)在肝脏局灶性结节增生(FNH)影像学诊断中的应用价值。资料与方法 18例FNH患者均进行16层螺旋CTA检查,CTA后处理技术包括多平面重组(MPR)、最大密度投影(MIP)和容积再现(VR)等。结果 18例患者共发现22个FNH病灶(直径2~10cm,平均4.2cm),2例为多发FNH,9个病灶较小(直径≤3cm)。动脉期明显强化,延迟期呈等或稍高密度,发现30支增粗供血动脉,34支引流静脉。22个病灶中12个发现中心瘢痕和纤维间隔,8个存在假包膜,15个具有占位效应。结论 16层螺旋CTA能充分显示FNH粗大供血动脉、引流静脉、假包膜、中心瘢痕、纤维间隔及网格状强化等特征性影像学表现,具有重要诊断价值。
Objective To study the clinical value of 16-Slice CT angiography(CTA)in Focal Nodular Hyperlasia(FNH).Materials and Methods Eighteen patients with FNH performed CT examination using 16-slice CT scanner.CTA images were reconstructed using multiplanar reconstruction(MPR),interactive maximum-intensity-projection(MIP),and volume-rendering(VR)techniques.Results CT detected 22 tumors in 18 patients(mean diameter,4.2 cm;range,2-10 cm).Two patients had multiple FNH lesions.Nine lesions were small than 3 cm.FNH lesion showed hypervascular mass and hyper-attenuation to liver on arerial phase and iso-attenuation or hypo-attenuation to liver on delayed phase.Thirty dilated feeding arteries and 34 draining veins were detected on CTA.Central scar and fibrous septum were observed in 12of 22 lesions.Eight lesions had pseudocapsules and 15 lesions had mass effect.Conclusion 16-slice CTA with advanced image processing is a powerful tool to identify the imaging features of FNH,including the feeding arteries and draining veins,pseudocapsule and central scar.
