摘要
目的:探讨血清二胺氧化酶(Diamine oxidase,DAO)水平在新生儿缺氧缺血性脑病(Hypoxic ischemic encephalopathy,HIE)早期诊断、临床分度及合并肠屏障功能损害早期评价中的应用价值。方法:检测41例足月HIE患儿在生后3 d内血清DAO水平,并在生后7 d内进行HIE临床分度,以同期9例正常新生儿为对照组。结果:HIE组血清DAO为(11.456±2.227)U/ml,明显高于对照组(6.277±2.431)U/ml(P<0.01);HIE组随临床分度的加重,血清DAO水平逐渐增高(P<0.01);中重度HIE组血清DAO的异常发生率92.9%高于轻度HIE组63.0%,其差异有统计学意义(P<0.05);中重度HIE患儿中小于胎龄儿组血清DAO的异常发生率80.0%高于适于胎龄儿组55.6%,其差异有统计学意义(P<0.05)。结论:监测窒息新生儿出生后3d内血清DAO水平,在HIE的早期诊断、临床分度及其合并肠屏障功能损害的早期评价中具有一定的应用价值。
Objective:To investigate the value of determining serum diamine oxidase in early diagnosis and grading of the hypoxic ischemic encephalopathy (HIE)and early evaluation of HIE combined with intestines barrier function injury. Methods : Serum DAO activities were measured in 41 neonates of full-term HIE during the first three days of life. Each patient’s clinical grading was assessed at 7 days after birth , and 9 healthy neonates in the same period were randomly chosen as control group. Results:The serum DAO in HIE group group were (11.456±2.227)U/ml,significantly higher than ( 6.277±2.431 ) U/ml in control group ( P0.01 ) .With increase of clinical degree , serum DAO gradually increased ( P0.01 ) . The abnormal rate of serum DAO of severe HIE group was 92.9% , significant higher than 63.0% in mild group ( P0.05 ) . The abnormal rate of serum DAO in Light-For-Dates infants were 80.0%,higher than 55.6% in severe HIE group of infants appropriate for gestational age (P 0.05). Conclusion:Measurement of serum DAO at 3 days after birth is valuable in early diagnosis and grading of HIE and early evaluation of HIE combined with Intestines barrier function injury.
