收缩性、舒张性心力衰竭血浆BNP与vWF水平的对比研究 被引量:9

摘要 目的探讨收缩性和舒张性心力衰竭患者血浆中人脑利钠肽(BNP)及血管性血友病因子(vWF)的变化、相关性和临床意义。方法选择心力衰竭患者44例,并收集22个正常人作对照,测量血浆BNP及Vwf,并按收缩性、舒张性心衰及心功能分级进行比较分析。结果①心力衰竭(HF)患者血浆BNP、wWF增高(P<0.001),收缩性心力衰竭(SHF)组高于舒张性心力衰竭(DHF)组,心功能Ⅲ~Ⅳ级组高于Ⅰ~Ⅱ级组(P<0.05);②BNP与vWF呈正相关(rs=0.567,P=0.001)。结论①BNP评价收缩性心衰较舒张性心衰更敏感。②BNP、vWF均可反映收缩性及舒张笥心力衰竭程度。 【Objective】To observe the characteristics of brain natriuretic peptide(BNP) and von Willebrand factor(vWF) in systolic or diastolic heart failure(SHF or DHF) and to observe whether there was a relation between vWF and BNP in HF.【Methods】Totally 44 cases with congestive HF(CHF) and 22 normal cases acted as agematched control were included in this research.The concentration of BNP and vWF was measured,whose levels were compared in patients with systolic or diastolic heart failure and different heart function.【Results】The plasma vWF and BNP levels(but not sTM) were all increased significantly in patients with HF compared to those in the control group,they were higher in patients with SHF than in patients with DHF,higher in patients with grade Ⅲ~Ⅳ heart function than in those with gradeⅠ~Ⅱ heart function(P 0.05).There were significantly positive correlations between BNP and vWF(rs=0.567,P0.001).【Conclusion】Plasma BNP is an important biomarker of heart failure,and it is more sensitive in SHF than DHF.Both BNP and vWF could reflect the degree of the heart failure
