摘要
目的探讨自体神经移植、神经导管技术修复颅内段动眼神经缺损的效果。方法22只家猫随机分为3组,神经移植组10只,导管组10只,对照组2只。右侧动眼神经制作4mm缺损后,分别用自体腓肠神经移植、PLGA+BDNF/CNTF修复,对照组不修复。14周时处死动物,取远端再生神经,用光镜、电镜观察及轴突图像分析评估修复效果。结果术后14周时,神经移植组、导管组各有8只猫术侧神经功能基本恢复,神经连续性恢复;对照组无恢复。神经移植组远端再生神经有髓纤维数目为(12032±999)个,平均轴突直径为(5.19±0.38)μm;导管组远端再生神经有髓纤维数目(10274±881),平均轴突直径为(4.78±0.32)μm;两组间纤维数目和轴突直径差异有显著性(P<0.05)。结论自体腓肠神经移植及PLGA+BDNF/CNTF导管法均能有效修复猫动眼神经缺损,自体腓肠神经移植法修复效果好于PLGA+BDNF/CNTF导管法。
【Objective】To evaluate the effects of nerve regeneration when the oculomotor nerve defects were repaired with autologous nerve graft and PLGA conduit in cats.【Methods】22 cats were randomly divided into three groups,nerve graft group(n=10),conduit group(n=10),control group(n=2).The cistern segment of the right ocu-lomotor nerve was transected,a 4 mm nerve defect was wade.The nerve defects in nerve graft group were repaired with autologous sural nerve graft,in conduit group were repaired with PLGA conduit and BDNF/CNTF mixture,while no repairing were made in the control group.At 14 weeks after surgery,the regenerating nerves were documented histologically and morphometrically.【Results】At 14 weeks after surgery,in nerve graft group and conduit group,there were both 8 cats showed gradual improvement with time of the function of oculomotor nerves,while no improvement were found in control groups.The anatomical continuity of the nerves was recovered,the nerve regenera-tion in nerve graft group and conduit group were confirmed by histological examination,while no recovery were found in control groups.The mean nerve fiber number of the distal stump were(12 032±999) in nerve graft group,(10274±881) in conduit group.The mean axonal diameter of the distal stump were(5.19±0.38)μm in nerve graft group,(4.78±0.32)μm in conduit group,The nerve fiber number and mean axonal diameter in nerve graft group were higher than those in conduit group(P〈 0.05).【Conclusions】Both autologous nerve graft and PLGA conduit can successfully repair the oculomotor nerve defects in cat.The repair effects of autologous sural nerve graft were much better than using BDNF/CNTF mixture in PLGA conduit while repairing the nerve defects of oculomotor nerve.
