老年胆囊结石120例手术治疗临床分析

Clinical analysis of surgical treatment of 120 elderly patients with gallstone

目的总结老年胆囊结石手术治疗经验。方法回顾性分析该院收治的120例胆囊结石患者的临床资料,分析治疗效果。结果该组120例患者,行急诊手术14例,择期手术106例,其中单纯胆囊切除92例,胆囊造瘘术2例,胆囊大部切除5例,胆囊切除加胆总管探查、T管引流21例。该组患者无1例死亡。术后并发症有肺部感染2例,切口感染5例,2例出现胆瘘,经积极对症治疗后均治愈出院。结论老年人胆囊结石手术治疗应完善术前的准备,选择合适的手术时机和手术方式,积极地治疗合并症,这样可以有效地减少手术的风险及术后并发症的发生率。

【Objective】To summarize the experience of the elderly gallstone surgery.【Methods】A retrospective analysis was conducted on clinical data of 120 cases gallstone patients, and therapeutic effect was analyzed.【Results】14 cases were performed emergency operation in total 120 patients. There were 92 cases of simple cholecystectomy, 2 cases of cholecystostomy, 5 cases of subtotal cholecystectomy, 21 cases of cholecystectomy plus choledochotomy with exploration and T-tube drainage in 106 patients with elective surgery. Postoperative complications included 2 cases of pulmonary infection, 5 cases of wound infection, 2 cases of biliary fistula, and no death. All patients were cured after symptomatic treatment. 【Conclusion】Improving preoperative preparation, selecting the appropriate opportunity of surgery and surgical approach, actively curing complications, can effectively reduce the risk of surgery and postoperative complication rates in the treatment of elderly patients with gallstone.
