流式细胞术检测HLA-B27抗原的表达及临床意义

Detection and clinical significance of HLA-B27 antigen by flow cytometry

目的评价流式细胞仪法检测HLA-B27抗原对强直性脊柱炎(AS)、急性前葡萄膜炎(AAU)的临床诊断价值。方法用流式细胞仪检测85例AS患者、40例AAU患者、90例背痛和腰腿痛患者以及80例健康对照组的HLA-B27抗原表达。结果 AS组HLA-B27检测阳性率(91.8%)高于背及腰腿痛组(5.6%)和健康体检组(3.8%);AAU组HLA-B27检测阳性率(52.5%)高于健康体检组;背及腰腿痛组HLA-B27检测阳性率与健康对照组比较无意义。结论流式细胞仪法检测HLA-B27抗原对AS的临床诊断有较高敏感度和特异度,是一种准确、有效的实验诊断方法 ;对AAU患者进行HLA-B27检测,对评价其临床特征和预后有一定价值。

[Objective] To evaluate the clinical diagnostic value of HLA-B27 in patients of ankylosing spondylitis（AS）and acute anterior uveitis（AAU）by flow cytometry（FCM）.[Methods] Using FCM,HLA-B27 was determined in 85 AS patients,40 AAU patients,90 backache and backleg pain patients,and 80 controls.[Results] The positive rate of HLA-B27 in AS patients was 91.8% and was higher than that of backache and backleg pain patients（5.6%）or controls（3.8%）.The positive rate of HLA-B27 in AAU patients（52.5%）was higher than that of controls.There was no significant difference between the backache and backleg pain patients and controls for the positive rate of HLA-B27.[Conclusions] It is suggested that the determination of HLA-B27 for the diagnosis of AS is an accurate and effective diagnostic method as its highly sensitivity and specificity.Detecting HLA-B27 in the patients with AAU is helpful to evaluate the clinical characteristics and the prognosis of AAU.
