带旋髂深血管蒂髂骨瓣移植治疗青壮年股骨颈骨折

Treatment of femoral neck fractures in young adults with pedicled vascularized iliac crest

目的探讨用带旋髂深血管蒂髂骨瓣移植治疗青壮年股骨颈骨折的方法和疗效。方法采用带旋髂深血管蒂髂骨瓣移植加螺钉内固定，治疗青壮年股骨颈骨折１４例。结果术后随访１年～４年６个月，１３例骨折均愈合，未发生股骨头缺血性坏死，患侧髋关节功能恢复良好。结论该术式能提高青壮年股骨颈骨折的愈合率，避免发生骨不连和股骨头缺血性坏死等严重并发症。

Objective To study if the treatment of femoral neck fractures in young and middle aged adults could be improved with pedicled bone flap transposition of deep iliac circumflex blood vessel. Methods 14 cases of femoral neck fractures in young and middle aged adults were treated by multiple screws and iliac osteoperiosteal flap pedicled with deep iliac circumflex blood vessel. Results 13 cases showed excellent outcomes after following up for 1～ 4 5 years. The femoral neck fractures have been healed. There was no ischemic necrosis of femoral head. Their functions on the hips were satisfactory. Conclusion The technique can improve the healing of the femoral neck fractures in the young and middle aged adults and reduce complications in bone formation and in avascular necrosis of femoral head.
