拔除下颌切牙配合邻面去釉矫治错的体会

Clinical experience of orthodontic in the treatment of lower incisor extraction

目的探讨正畸拔除下颌切牙的诊治特点和如何配合邻面去釉调整bolton指数。方法选择设计拔除下颌切牙矫治错畸形患者16例,其中单纯拔除1个下颌切牙者9例,下颌拔除切牙、上颌拔除第一双尖牙者7例,配合邻面去釉使Bolton指数接近正常值。结果单纯拔除一个下颌切牙的9例患者,平均疗程(8.6±0.5)个月。下颌拔除切牙、上颌拔除双尖牙的7例患者,平均疗程(21.5±1)个月。结论单纯拔除一个下切牙者可缩短疗程,但应尽量减少邻面去釉。设计为下颌拔除2个切牙,上颌拔除第一双尖牙时,减数模式比减数模式可减少邻面去釉。

Objective To explore the indication of orthotic malocclusion with lower incisor extraction and the coordinated way of interproximal enamel stripping to adjust bolton index.Methods 16 patients with malocclu-sion were enrolled and divided into two groups.One group of 9 patients was treated with extraction of one lower inci-sor,and the other group of 7 patients is treated with extraction of lower incisors and upper first premolars.Inter-proxi-mal enamel stripping was used to adjust the Bolton index to access normal value.Results The treatment course of the first group is 8.6 ±0.5 months.The course of the second group is 21.5 ±1 months.Conclusion The orthodontic course with extraction of one lower incisor may be accomplished with less time,compared with that of extraction of lower incisors and upper first premolars.However,inter-proximal enamel should be stripped as little as possible.More-over,compared with extraction,less inter-proximal enamel was stripped when were extracted.
