B超监测下宫腔镜治疗56例子宫粘膜肌瘤临床分析

The clinical analysis of hysteroscopy treatment of submucous myoma on B ultrasonic monitoring

目的探讨B超监护下宫腔镜切除子宫粘膜下肌瘤的临床疗效及安全性。方法 B超监护下采用宫腔镜切除术治疗粘膜下子宫肌瘤。56例患者中:0型29例,Ⅰ型20例,Ⅱ型7例。术后随访3～24个月。根据术后月经改善情况、剩余肌瘤有无增长、是否再次手术等将疗效分为满意和不满意。对结果进行分析。结果应用宫腔镜手术治疗粘膜下子宫肌瘤,术后满意率为96.2%;0型、Ⅰ型术后满意率达100%,Ⅱ型子宫肌瘤术后满意率75%。术后随访,残余肌瘤不再增长。3例要求生育者妊娠。全部手术无一例并发症发生。结论宫腔镜手术治疗粘膜下子宫肌瘤疗效好,正确选择手术适应证,严格B超监测可提高手术安全性和术后疗效。

Objective To explore the clinical efficacy and safety of hysteroscopy treatment for submucous myoma on B ultrasonic monitoring.Methods The submucous myoma was all removed by hysteroscopy.Among 56 cases with submucosal fibroids,the 0 type were 29 cases,Ⅰ type 20 cases,Ⅱ type 7 cases.All cases were followed up for 3-24 months.The effects of treatment were divided into satisfactory and unsatisfactory according to whether improvements in postoperative period.Results The total satisfaction after hysteroscopy surgery was 96.2%.The post-operative satisfaction rate was 100% for the 0 and Ⅰ type,Ⅱ type 75%.The residual fibroids did not continue to grow.3 cases required fertility conceived.There was no complication in all the operation.Conclusion It is a good method for hysteroscopy to treat submucosai hystemmyoma.The effects and safety of the operation could be ensured by selecting proper patients and strict ultrasound monitor in the operation.
