腰椎间盘脱出合并马尾综合征40例临床分析

Clinical analysis for lumbar disc herniation combined with cauda equina syndrom in 40 cases

目的:探讨急性腰椎间盘脱出伴马尾神经损伤的手术疗效。方法:回顾性分析我院2006年4月～2009年2月对收治的腰椎间盘急性脱出造成的马尾神经损伤患者40例,随机分为两组,治疗组采用髓核摘除术、椎管减压术,对照组应用牵引等物理治疗,评价两组临床治疗效果及其术后恢复情况。结果:手术治疗组患者总有效率为100%,明显优于对照组的60%(P<0.05)。本组40例患者马尾神经功能完全恢复32例,不完全恢复8例。结论:急性腰椎间盘脱出伴马尾神经损伤应尽早手术治疗,术后中西医结合治疗对预后有很大帮助。

Objective：To discuss the surgery efficacy of lumbar disc herniation combined with cauda equina syndrome.Methods：A retrospective analysis of 40 patients with lumbar disc herniation and nerve injury of cauda equine from April 2006 to February 2009 in our hospital,the patients were divided into treatment group and control group,the treatment group was used myelin nuclear extirpation and canalis spinalis decompression,but was traction in the control group.The clinical efficacy and recovery after treatment in two groups were analyzed.Results：There was a significant difference about the total effective rate in treatment group（100%） and the control group（60%）（P0.05）.Among 40 patients,32 cases of cauda equina function returned,8 cases of incomplete recovery.Conclusion：Acute lumbar disc herniation combined with injury of cauda equine should be treated by surgery,the prognosis is good under the treatment of integrative medicine.
