血必净注射液对急性重度有机磷中毒患者心肌保护作用的研究

Myocardial protection of Xuebijing injection on acute severe organophosphorus pesticide poisoning patients

目的观察血必净注射液对急性重度有机磷中毒患者血清心肌的酶值的影响,探讨其对心肌的保护作用。方法将104例急性重度有机磷中毒患者,按治疗方法不同,分为对照组(50例)和实验组(54例)。对照组采用常规治疗方法(包括洗胃、阿托品、氯解磷定、补液、利尿等),实验组在常规治疗基础上,加用血必净50mL+生理盐水100mL,2次/d静点,连用7d;分别于入院后即刻、第1天、第4天采静脉血,测量血清心肌酶值,包括:血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、肌钙蛋白I(cTnI)、乳酸脱氢酶(LDH),并进行对比,比较两组患者的死亡率。结果治疗后第1天及第4天,血清心肌酶值较治疗前明显下降(P<0.05),实验组较对照组亦明显下降(P<0.05);实验组死亡率低于对照组(P<0.05)。结论早期联合应用血必净注射液和常规治疗药物,可降低急性有机磷中毒患者的心肌损害,提高救治成功率,血必净治疗急性有机磷中毒值得临床推广。

Objective To investigate the influence of Xuebijing injection on myocardial enzyme levels of acute severe organophosphorus pesticide poisoning patients.Methods 104 patients with acute severe organophosphate poisoning were divided into control group（50 cases）and experimental group（54 cases）.Control group was treated with conventional treatment,and experimental group was treated with similar therapy besides intravenous injection of Xuebijing 50 mL,bid with 7 d.The venous blood was collected at 0,1st and 4th day after treatment separately,and the blood serum myocardial enzyme values were measured,the mortality rates of two groups were observed.Results Serum myocardial enzyme values decreased significantly at 1st and 4th day after treatment（P0.05）,and the values in experimental group decreased more significantly than control group（P0.05）.The mortality of experimental group was significantly lower than that of control group（P0.05）.Conclusion Xuebijing injection combined with routine therapies can reduce myocardial damage of acute organophosphate poisoning patients,and reduce patients＇ mortality.
