两种手术方式治疗精索静脉曲张的对比观察

Comparison of two kinds of surgical treatments for varicocele

目的比较观察经腹膜后开放手术和经腹腔镜精索静脉高位结扎术治疗精索静脉曲张的疗效。方法回顾性分析本院2006年12月~2008年11月治疗的92例精索静脉曲张患者的临床资料,其中50例经腹膜后小切口精索静脉结扎,42例经腹腔镜精索静脉结扎。比较两种手术方式治疗的手术时间、术后住院天数、住院费用以及手术对精液质量的影响、术后复发和并发症等情况。结果腹腔镜组双侧病变手术时间和术后住院天数明显少于开放手术组,差异有统计学意义（P（0.05）;腹腔镜组患者住院费用明显高于开放手术组,差异有统计学意义（P（0.05）;两种手术方式均能改善精液质量,但差异无统计学意义（P（0.05）;术后并发症的发生率2组比较无统计学意义（P（0.05）。结论腹腔镜治疗精索静脉曲张比开放手术具有创伤小、术后恢复快、住院时间短等优点,但是住院费用较高。

Objective To compare the value of internal spermatic vein high ligation by laparoscopy and open operation for varicocele.Methods 92 patients with VC in our hospital were treated in two different ways from December 2006 to November 2008,and their clinical data were retrospectively analyzed.Among these cases,50 were treated by operation,42 with laparoscopy.Results Laparoscopic method had a significant advantage in operation time,post operation stay in hospital over open operation method,but was more costly.Both methods could improve the quality of semen,with no significant difference.Conclusion Laparoscopic internal spermatic vein high ligation is much better than patent operation except for the cost.
