术中超声在脑出血手术中的应用价值研究

Value of Intraoperative Ultrasound in Hypertensive Intracerebral Hemorrhage Operation

[目的]探讨在高血压脑出血手术中超声的应用价值.[方法]38例高血压脑出血手术病例中采用术中超声辅助清除脑内血肿并与对照组进行术后残余血肿量、术后1个月预后的比较.[结果]研究组平均颅内残留血肿量为（4.5±1.7） mL,明显低于对照组[（10.7±4.6） mL],两组相比较差异有显著性（P〈0.05）.术后一个月两组病例预后结果无差异.[结论]采用术中超声技术不仅能准确定位血肿位置、彻底清除脑内血肿、保护周边正常组织不受损伤,更重要的是有了术中超声定位技术保障后就能给予手术者以足够信心,从而提高了手术疗效.

[Objective] To explore the value of intraoperative ultrasound in hypertensive intracerebral hemorrhage operation. [Methods] Thirty eight eases of hypertensive intraeerehral hemorrhage underwent intraoperative ultrasound-assisted cerebral hematoma resection. The volume of remained hematoma after operation and the prognosis one month after operation were compared between hypertensive intracerebral hemorrhage group and the control group. [Results] The average volume of remained intracerebral hematoma in the study group was 4.5 ±1.7mL which was significantly lower than that in the control group（10. 7 ±4.6mL）（ P 〈0.05）. There was no difference in the prognosis one month after operation between two groups. [Conclusion] Intrao- perative ultrasound can confirm the location of hematoma accurately, and remove intracerebral hematoma completely, and protect the peripheral normal tissue. More importantly, the technological support of intraoperative ultrasound may give operator sufficient confidence to improve the effect of operations.
