早期胃癌168例胃镜分析

Gastroscopy in the diagnosis of early gastric cancer:a retrospective analysis of 168 cases

目的:探讨早期胃癌的胃镜诊断特点。方法:回顾性分析经胃镜检查并经病理确诊的2089例胃癌中的168例早期胃癌的临床、胃镜、病理资料。结果:胃镜下诊断为I型39例(23.21%),IIa型17例(10.12%),IIb型15例(8.93%),IIc型55例(32.74%),III型42例(25.00%);病理分型中,低分化腺癌86例(51.19%),管状腺癌53例(31.55%),乳头状腺癌11例(6.55%),黏液腺癌8例(4.76%),印戒细胞癌7例(4.17%),未分化癌2例(1.19%),特殊型癌1例(0.59%)。结论:胃镜检查的重要价值之一在于对胃癌的早期诊断,对有消化道症状,或者有胃癌家族史者,建议对其积极进行胃镜检查并定期随访,以便早发现、早治疗,提高治愈率。

Objective：To investigate the characteristics of gastroscopy in the diagnosis of early gastric cancer.Methods：One hundred and sixty-eight patients of EGC were selected from 2 089 patients with gastric cancer who were confirmed by gastroscopy and pathological examination.The results of clinical and pathological examination and gastroscopy were analyzed using retrospective analysis.Results：Gastroscopy showed that 39 patients had type I gastric cancer（23.21%） ;17 patients had type IIa gastric cancer（10.12%） ;15 patients had type IIb gastric cancer（8.93%） ;55 patients had type IIc gastric cancer（32.74%） ;42 patients had type III gastric cancer（25.00%）.Pathological examination found that 86 patients had poorly differentiated adenocarcinoma（51.19%） ;53 patients had tubular adenocarcinoma（31.55%） ;11 patients had papillary carcinoma（6.55%） ;8 patients had mucinous adenocarcinoma（4.76%） ;7 patients had signet ring cell cancer（4.17%）,2 patients had undifferentiated carcinoma（1.19%）,1 patient had specific type of cancer（0.59%）.Conclusion：One of the important values of gastroscopy is the early diagnosis of gastric cancer.Gastroscopy and periodic follow-up are recommended for the people who have digestive symptoms or family history of gastric cancer,which will help to detect and treat gastric cancer at early stage and increase the curing rate.
