造影剂外渗应用皮肤创面无机诱导活性敷料的观察护理

Observation and Nursing of Treatment of Extravasation of Barium Meal by Applying Dermlin

目的观察造影剂外渗应用皮肤创面无机诱导活性敷料止痛和消肿效果。方法将CT冠状动脉造影检查注射部位发生外渗的45例患者,随机分为3组,皮肤创面无机诱导活性敷料组(A组)、硫酸镁湿敷组(B组)、金黄散湿敷组(C组),每日2次,连续3 d,分别记录3组不同治疗方法后肿胀部位止痛起效时间、疼痛缓解程度和消肿情况。结果 A、B、C 3组止痛起效平均时间分别为(17.4±6.2)、(26.5±8.7)、(50.5±5.0)min,A组较B、C组止痛起效时间快,3组间比较,差异有统计学意义(P<0.01);治疗24 h后,A组86.67%的患者疼痛消失,13.33%的患者有轻度疼痛,B组13.33%无痛,86.66%患者有轻、中度疼痛,C组46.67%无痛,53.33%患者有轻、中度疼痛;治疗48 h后,3组肿胀疗效比较,A组较B、C组消肿快,差异有统计学意义(P<0.01)。结论皮肤创面无机诱导活性敷料用于造影剂外渗止痛、消肿作用快,减轻患者痛苦。

OBJECTIVE To observe dressing pain and swelling results of dermlin application for Extravasation of contrast agent.METHODS A total of 45 patients,who occurred extravasation with CT angiography on coronary artery in injection site,were randomly divided into three groups： dermlin group;salamarum wet packing group;agreeable golden powder wet packing group.2 times a day,continuous for 3 days.Pain relief and swelling situation in three different parts were recorded after pain treatment after onset of swelling.RESULTS The average time of pain onset after application of dermlin： 17.4±6.2 min;agreeable golden-26.5 ± 8.7 min;salamarum solution-50.5 ± 5.0min.Dermlin acted faster than inorganic golden powder and salamarum.Onset time of analgesia among the three groups showed significant difference（P0.01）.After 24 h treatment,86.67% of patients,who applicated dermlin,had no pain and 13.33% of the patients had mild pain;in salamarum wet packing group 13.33% painless,86.66% patients had mild pain and moderate pain;in golden powder group 46.67% painless,53.33% of the patients had mild pain and moderate pain;After 48 h treatment three groups compared swelling effect,dermlin group effect faster,than other two groups.The three groups was statistically significant,（P0.01）.CONCLUSION Dermlin can be applied for the pain relief and reduce the swelling quickly.Dermlin can reduce the suffering of the patients.
