中枢神经系统感染病原菌的培养及耐药性分析

摘要 目的了解中枢神经系统感染病原菌的分布及耐药状况。方法采用英国先德ARIS 2X系统进行病原菌培养、鉴定及药敏测试。结果 780份脑脊液标本,分离出病原菌152株,阳性率19.49%,其中革兰阳性菌106株(69.74%),革兰阴性菌45株(29.61%),真菌1株(0.65%);耐甲氧西林金黄色葡萄球菌和耐甲氧西林凝固酶阴性葡萄球菌的检出率分别为66.67%和73.61%,未发现耐万古霉素的葡萄菌、肠球菌、万古霉素、奎奴普丁/达福普汀对革兰阳性球菌均有较高的抗菌活性;革兰阴性菌对常用药物头孢他啶、头孢曲松的敏感性较差,亚胺硫霉素对革兰阴性菌有较好的抗菌活性。结论中枢神经系统感染最常见的病原菌为凝固酶阴性葡萄球菌;本组结果为中枢神经系统感染的预防和经验性抗菌药物治疗提供可靠的数据资料。 Objective To determine the distribution and resistance of bacteria isolated from CSF in meningitis patients.Methods The bacteria identification and the antimicrobial susceptibility were assayed by automat ARIX 2X system.Results The positive rate of pathogens was 20.88% from 780 cerebrospinal fluid specimens.There were 152 strains of pathogens which include 106 Gram posi-tives,45 Gram negatives band 1 fungi.66.67 % and 73.61 % of S.aur and CNS were meticillin re-sistant.No Staphylococcus and Enterococcus strain was resistant to VAN.All the Gram-positive were sensitive to VAN、SYN;the Gram-negative bacteria were resistant to routine antibiotics such as CAZ、 CRO,while they have high percentage of susceptibility IPM.Conclusion CNS are the predominant organisms from CSF of central nervous system infection patients.Data collected in present study pro-vide valuable information for prophylactic and empirical antibiotic use in intracranial infection.
