老年COPD患者稳定期肠内营养治疗的临床研究

Clinical Study of Enteral Nutrition in Elderly Patients with Stable COPD

目的探讨肠内营养对老年COPD患者营养状态、血清炎性因子及心功能的影响。方法以41例老年COPD稳定期患者为研究对象,将患者随机分为两组,两组在常规治疗同时,其中一组给予肠内营养,另一组自由饮食。治疗前后均检测各营养指标,用放免发测TNF-α、IL-6,用心脏超声检测左室射血分数LVEF。结果肠内营养组患者在治疗后各营养指标均有改善,炎性因子水平均有降低,心功能均有所改善,治疗前后比较有统计学意义(P<0.05)。自由饮食组中,各营养指标改善不明显,炎性因子水平及心功能改善不大,前后比较无统计学意义(P>0.05)。结论老年COPD稳定期患者,在常规治疗同时重视肠内营养治疗,可以改善患者营养状况从而改善功能免疫,调节炎性因子水平,进一步改善患者心功能。

Objective To investigate enteral nutrition on nutritional status of elderly patients with COPD,serum inflammatory factors and cardiac function.Methods Of 41 cases of elderly patients with stable COPD for the study,patients were randomly divided into two groups,while the conventional treatment groups,one group received enteral nutrition,and the other free diet.Nutrition before and after treatment were detected in the index,measured by RIA hair TNF-α,IL-6,detection of left ventricular ejection fraction by echocardiography LVEF.Results Enteral nutrition in the treatment of patients were improved after the nutrition indicators,have lower levels of inflammatory cytokines and cardiac function were improved after treatment was statistically significant（P0.05）.Free diet group,no obvious improvement of the nutritional indices,inflammatory cytokines and cardiac function,no significant difference before and after（P0.05）.Conclusion Elderly patients with stable COPD,in the conventional therapy while focusing on enteral nutrition therapy can improve the nutritional status of patients to improve immune function,regulate inflammatory cytokines,further improve cardiac function.
