牙周健康状况对中晚期妊娠妇女血清hs-CRP的影响 被引量:6

摘要 目的:研究牙龈炎、牙周炎对中晚期妊娠妇女血清超敏C反应蛋白(hs-CRP)的影响。方法:选择84例中晚期妊娠妇女为研究对象,其中牙龈炎30例、牙周炎24例,健康者30例作为对照。分别检查各受试者牙龈指数(GI)、牙周探诊深度(PD)、临床附着丧失(CAL)、龈沟出血指数(SBI)、牙龈炎或牙周炎阳性位点率等;同时抽取空腹肘静脉血5mL,以胶乳增强的免疫比浊法测定血清hs-CRP水平。结果:牙龈炎组GI、阳性位点率、hs-CRP与对照组比较有统计学差异(P<0.05),牙周炎组与对照组、牙龈炎组之间,在各牙周临床指数和hs-CRP均有着统计学差异(P<0.05);hs-CRP与牙周临床指数GI、PD、CAL、SBI、阳性位点率之间相关系数分别为0.783,0.579,0.362,0.772、0.673;阳性位点率(R2=0.60)、SBI(R2=0.45)对hs-CRP线性回归的贡献较大。结论:①hs-CRP水平与GI、PD、CAL、SBI、牙龈炎或牙周炎阳性位点率等牙周临床指数之间存在正相关关系;②阳性位点率和SBI对hs-CRP线性回归的贡献较大,提示牙龈炎、牙周炎在妊娠女性口腔所累及的范围与炎性程度对血清CRP水平的影响较大,牙龈炎、牙周炎与妊娠中晚期的血清CRP水平存在某种关联。 AIM:To investigate the influence of maternal periodontal status on the serum hyper sensitive-C reactive protein ( hs-CRP) levels in median or late pregnancy. METHODS:Based on the periodontal health status,84 women in median or late pregnancy were enrolled and categorized as periodontitis ( PG,n = 24) ,gingivitis ( GG,n =30) ,and control group ( CG,n =30). The clinical periodontal measurements,including gingival index ( GI) ,probing depth ( PD) ,clinical attachment loss ( CAL) ,sulcus bleeding index ( SBI) and the positive sites rate of gingivitis or periodontitis were checked and recorded. Meanwhile,fasting peripheral blood sample was collected by venipuncture. The serum hs-CRP level was quantified by latex enhanced nephelometry. RESULTS:The parameters including GI,serum hs-CRP level positive sites rate and hs-CRP level had significant differences between GG and CG ( P 〈 0. 05). Meanwhile,serum hs-CRP level and each of the clinical parameters were significantly higher in PG group than those in GG or CG group ( P 〈0. 05). The hs-CRP level had positive correlation with GI,PD,CAL,SBI,and positive site rate ( r =0.783,0.579,0.362,0.772,0.673,respectively). By regression analysis,we also found that the contributions of positive site rate ( R2 = 0. 60) and SBI ( R2 = 0. 45) to hs-CRP linear regression were higher than other parameters.CONCLUSION:The hs-CRP level had positive correlation with GI,PD,CAL,SBI,and positive site rate. The contributions of positive site rate and SBI to hs-CRP linear regression were higher,which suggested that the range and severity of gingivitis and periodontitis were possibly associated with elevated serum CRP level to some extent.
