应用卡孕栓联合缩宫素用于预防产后出血的临床分析

Clinical analysis of carprost combined oxytocin for the prevention of postpartum hemorrhage

目的:观察卡孕栓联合缩宫素用于预防产后出血的临床效果。方法:选择本院2007年8月～2010年8月足月正常妊娠阴道分娩产妇600例,随机分为A、B、C3组各200例,A组,卡孕栓直肠给药联合缩宫素静脉点滴;B组,卡孕栓直肠给药联合缩宫素肌内注射;C组,单用缩宫素肌内注射。观察3组产妇第三产程时间、产后2h的出血量情况。结果:A组和B组联合用药较C组在缩短第三产程时间,减少产后出血量方面均有显著改善,差异有统计学意义(P<0.05);A组较B组在缩短第三产程时间,减少产后出血量方面,差异有统计学意义(P<0.05)。结论:卡孕栓联合缩宫素在预防产后出血中较单一应用缩宫素效果好,且卡孕栓联合缩宫素静脉点滴较卡孕栓联合缩宫素肌内注射效果好,且用药安全,起效快,维持时间长,副作用小,值得在临床推广应用。

Objective： To observe carprost thrombectomy oxytocin for the prevention of postpartum hemorrhage. Methods： From August 2007 to August 2010 the maternal vaginal delivery at term normal pregnancy of 600 cases were randomly divided to three groups A, B, C, each group 200 patients, A group, the united carprost suppository rectal administration intravenous oxytocin. B group, carprost suppository rectal administration of oxytocin intramuscularly combined. C group, single intramuscular injection of oxytocin. Observed three groups of maternal third stage of labor time, 2 hours post-partum bleeding situation. Results： Group A and group B combined treatment compared with group C in the shorter third stage of labor time, reduce postpartum bleeding quantity had a significant improvement, the difference was significant （P0.05）, group A than in the group B in the shorter third stage of labor time to reduce the amount of postpartum hemorrhage, there were significant difference （P0.05）. Conclusion： Carprost thrombectomy oxytocin in preventing postpartum hemorrhage is a more effective single application of oxytocin, and the card is pregnant thrombectomy compared with intravenous oxytocin card intramuscular injection of progesterone suppository combined effect of oxytocin and drug safety, rapid onset, maintenance long time, side effects, in clinical application.
