真菌性角膜溃疡治疗临床分析

Clinical analysis of treatment of fungal corneal ulcer

目的：探讨真菌性角膜溃疡的临床表现及诊疗特点.方法：对我院确诊的85例90眼真菌性角膜溃疡,比较不同面积溃疡者病情特点;给予抗真菌规范药物治疗,对角膜溃疡面积大,药物效果差者,给于结膜瓣遮盖术,探讨治疗效果.结果：角膜溃疡面积＞6mm2者,前房积脓、基质脓疡及眼压方面,较溃疡面积＜6mm2者严重,差异具有统计学意义,P＜0.05.治愈45眼,好转33眼,无效12眼.其中6例行结膜瓣遮盖术,2例行眼内容剜出术.结论：真菌性角膜溃疡早期明确诊断,给予积极有效的药物治疗,进行必要的手术治疗,可预防穿孔,取得良好效果.

Objective：To evaluate the clinical performance and the features of diagnosis and treatment of fungal corneal ulcer. Methods：In 85 cases of fungal corneal ulcers including 90 eyes, we compared the characteristic of ulcers of different sizes. All patients were gave standard antifungal therapy. The corneal ulcers of large area was gave the conjunctival flap if the effects of grug was bad. To explore the therapeutic effect of treatment. Results z Hypopy- on, stromal abscess and intraocular pressure in the corneal ulcer whose area is beyond 6ram2 was more serious than the the corneal ulcer whose area is less than 6mm2. The difference was significant,P^0.05.45 cases was cure. 33 was improved. 12 cases was void. 6 cases was gave conjunctival flap. 2 cases was gave the enucleatlon of the content of eye. Conclusion：It can prevent perforation and got good effeaey that the fungal corneal ulcer was diagnosed early,gave effective treatment of drug and made the necessary surgery .
