Experience in Treament of 156 Cases with Difficult Cholecystectomy

目的：总结本院开腹切除困难胆囊的手术经验.方法：回顾性分析我院2001年1月-2009年12月,156例困难胆囊经开腹胆囊切除的病例资料.结果：所有患者均治愈出院,无死亡.手术时间90-130min,术后漏胆3例二周内自行闭合.术后随访125例,无胆管狭窄、胆总管残留结石及＂小胆囊＂病例发生.结论：以＂蚕食法＂为主的手术方法对困难胆囊切除是安全有效的.

Objective： To summarize the experience in treatment of patients with difficult cholecystectomy. Methods： The data of 156 patients who underwent difficult cholecystectomy between Jan 2001 and Dec 2009 was retrospective collected. Results： All the operations were performed successful- ly. There was no death. The operative time was 90--130 min. 3 patients of bile leakage were cured two weeks after operations. 125 patients were fol- lowed up. There was no bile duct stricture, residual stones of common bile duct and ＂small gallbladder＂. Conclusion, It is safe and effective for difficult cholecyst removed by ＂nibble method＂.
