摘要
目的环瓜氨酸肽(cyclic citrullinated peptide,CCP)抗原与类风湿关节炎(rheumatoid arthritis,RA)的发病及致病过程密切相关,是RA原发致病靶抗原。文中通过观察三七总皂苷(panax notoginseng saponins,PNS)对RA患者外周血CCP抗原特异性T细胞(antigen special Tcell,AST)分泌干扰素(interferon,INF)-γ、白细胞介素-4(interleukin-4,IL-4)水平的影响,进一步阐明PNS治疗RA的机制。方法体外分离培养24例RA患者外周血淋巴细胞,分为4组处理观察,分别是RA空白对照组、RA CCP组、RA甲氨喋呤(MTX)对照组及RA PNS组。各组细胞体外培养72 h后,酶联免疫吸附(ELISA)法检测细胞培养上清中INF-γ及IL-4水平。结果与RA空白对照组相比,RACCP组分泌的INF-γ升高(P<0.05),IL-4降低(P<0.05),INF-γ/IL-4升高(P<0.01)。经PNS作用后,与RA CCP组相比,INF-γ有所下降(P<0.05),IL-4升高(P<0.05)。结论 PNS可抑制RA患者外周血CCP-AST分泌Th1型细胞因子INF-γ,促进Th2型细胞因子IL-4的分泌,对Th1/Th2型细胞因子网络失衡状态有一定调节作用,可能在治疗RA中发挥重要作用。
Objective It had been reported that panax notoginseng saponins(PNS)was effective on treating rheumatoid arthritis(RA).Cyclic citrullinated peptide(CCP)antigen,as target antigens,is on intimate terms with pathogenesy of RA.To study the effect of PNS on interferon-γ and interleukin-4 secreted by of CCP-AST RA.Methods Peripheral blood lymphocyte(PBL) from 24 RA patients were divided into four groups and cultured respectively in vitro.RA blank control group:PBL of RA patients were cultured and not interfered by drug;RA CCP group:PBL of RA patients were interfered by CCP(degree of enrichment 20 μg/ml);RA Methotrexate(MTX)control group:PBL of RA patients were interfered by CCP and MTX(each degree of enrichment 20 μg/ml);RA PNS group:PBL of RA patients were interfered by CCP and PNS(each degree of enrichment 20 μg/ml).After cultured respectively in vitro for 72h,the concentration of INF-γ and IL-4 were measured by ELISA.Results Compared with the concentration of INF-γ and IL-4 from RA blank control group,T cell of CCP group peripheral blood secreted more INF-γ(P0.05),lower IL-4(P0.05),and IFN-γ/IL-4 were up-regulated(P0.01).Compared with the concentration of INF-γ and IL-4 from RA CCP group,T cell of RA PNS group secreted lower INF-γ(P0.05),more IL-4(P0.05).Conclusion PNS can inhibit secretion of TH1 cytokines INF-γ,and promote secretion of TH2 cytokines IL-4.PNS may have effect on disequilibrium of TH1/TH2,which may have important effect on treatment of RA.
