小牛血去蛋白提取物凝胶和羟糖苷眼液对LASIK术后角膜上皮损伤的修复作用

Deproteinized calf blood extract gel and hydroxyl-glucoside eye drops on recovery of corneal epithelial injury after LASIK

目的对比小牛血去蛋白提取物凝胶(兴齐制药)和羟糖苷眼液(爱尔康)在准分子激光原位角膜磨镶术(laser in situkeratomileusis,LASIK)后对角膜上皮损伤修复作用的功能差异,为改善术后干眼的临床用药提供帮助。方法将2010年1月至4月在我院实施LASIK手术的76例(152眼)近视患者依据术后用药进行分组,Ⅰ组为小牛血去蛋白提取物凝胶组,Ⅱ组为羟糖苷眼液组,术前和术后1个月分别行角膜荧光素染色检查,记录泪膜破裂时间(break-up time,BUT)和角膜上皮染色评分,对比两种药物对LASIK术后角膜上皮损伤修复作用的差异。结果术前、术后的BUT[术后Ⅰ组:(5.61±2.72)s,Ⅱ组:(5.42±3.17)s]和角膜上皮染色评分,组内比较差异无统计学意义(P值分别为0.877和0.092),组间比较差异亦无统计学意义(P值分别为0.683和0.212)。进一步对术前轻度干眼对象进行比较,手术前后BUT变化量组间差异无统计学意义(P=0.107),角膜上皮染色评分变化量的组间差异亦无统计学意义(P=0.181)。结论对于实施了LASIK的患者,术后为防止干眼,使用两类药物具有等同疗效,在BUT的延长上,小牛去蛋白提取物凝胶具有优势;但对于术前轻度干眼的患者,羟糖苷眼液效果更佳。

Objective To investigate the difference of deproteinized calf blood extract gel（Xinqi Pharmaceutical）and hydroxyl-glucoside eye drops（Alcon）on recovery of corneal epithelial injury after laser in situ keratomileusis（LASIK）and to provide reference for drug treatment as improving postoperative symptoms of dry eye.Methods A total of 76 myopic patients（152 eyes）,which were taken LASIK in our hospital from January 2010 to April 2010,were divided into 2 groups according to the different postoperative drug treatment.Patients in group Ⅰ were treated with deproteinized calf blood extract gel,and patients in group Ⅱ were treated with hydroxyl-glucoside eye drops.Corneal fluorescein stains was performed,break-up time（BUT）of tear film was recorded and corneal epithelial staining was scored at pre-operation and postoperative 1 month.The differences of two drugs on recovery of corneal epithelial injury after LASIK were compared.Results Preoperative and postoperative corneal epithelial staining and BUT[Postoperative BUT were（5.61±2.72）s for group Ⅰand（5.42±3.17）s for group Ⅱ] had no difference in group Ⅰ or group Ⅱ（P=0.877;P=0.092）,either between the two groups（P=0.683;P=0.212）.After analyzing patients with light dry eye at pre-operation,we had found that change of BUT between pre-and post-operation was with no difference between the two groups（P=0.107）,either in change of corneal epithelial staining（P=0.181）.Conclusions For patients performed with LASIK,both two drug have equal curative efficacy to prevent dry eye.Deproteinized calf blood extract gel is good at prolonging BUT,but hydroxyl-glucoside eye drops is better for patients with light dry eye at pre-operation.
