血清内脂素、vaspin及脂联素与不同类型肥胖儿童代谢改变的相关性研究 被引量:1

The relationship between serum levels of visfatin,vaspin,adiponectin and metabolic disturbance in children with different types of obesity

摘要 目的了解肥胖儿童不同体脂分布状态下血清内脂素、vaspin及脂联素水平的差异,探讨其与儿童腹型肥胖及代谢的关系。方法测量78例腹型肥胖儿童、18例非腹型肥胖儿童及75例正常儿童的人体学指标及血脂、血糖、胰岛素水平,用酶联免疫吸附法(ELISA法)测定三组儿童血清内脂素、vaspin、脂联素浓度。结果腹型肥胖组、非腹型肥胖组、正常组血清内脂素分别为(68.91±21.85)ng/ml、(51.18±20.06)ng/ml、(34.83±14.09)ng/ml,差异有统计学意义(F=12.55,P<0.01);血清vaspin分别为(1 869±56)pg/ml、(1 543±42)pg/ml、(1 581±45)pg/ml,差异无统计学意义(P>0.05);血清脂联素分别为(7.54±2.12)μg/ml、(9.12±2.53)μg/ml、(10.21±3.34)μg/ml,差异有统计学意义(F=13.24,P<0.05)。内脂素水平与腰围(WC)、血压、空腹血糖(FPG)、服糖2 h后血糖(2hPG)、2 h胰岛素(2hINS)呈正相关;vaspin与FPG、2hPG、2hINS呈正相关;脂联素浓度与BMI、WC、FPG、2hPG、空腹胰岛素(FINS)呈负相关,而与高密度脂蛋白胆固醇(HDL-C)呈正相关。三种细胞因子均与FPG相关。结论内脂素、脂联素水平在儿童肥胖症中已发生改变,与腹型肥胖及糖代谢相关;vaspin参与糖代谢的调节。 Objective To study serum levels of visfatin,vaspin,and adiponectin in children with different fat distribution,and investigate the association between above three adipocytokines and metabolic disturbance in children with abdominal obesity.Methods Seventy-eight children with abdominal obesity and 18 children with non-abdominal obesity were recruited in the study,75 healthy children were studied as normal control.Anthropometric variables,serum lipids,serum glucose,and insulin were measured.Serum visfatin,vaspin,and serum adiponectin were examined by enzyme-linked immunosobent assay(ELISA).Results Levels of serum visfatin in abdominal obesity,non-abdominal obesity,and normal group were(68.91 ± 21.85)ng/ml,(51.18 ± 20.06)ng/ml,(34.83 ± 14.09)ng/ml,respectively,there were significant differences among them(F = 12.55,P 0.01).Serum levels of vaspin in these three group were(1 869 ± 56)pg/ml,(1 543 ± 42)pg/ml,and(1 581 ± 45)pg/ml,respectively,without significant difference(P 0.05).The concentrations of serum adiponectin were(7.54 ± 2.12)μg/ml,(9.12 ± 2.53)μg/ml,and(10.21 ± 3.34)μg/ml,respectively,the differences were significant(F = 13.24,P 0.05).Serum visfatin concentrations positively correlated with waist circumference(WC),blood pressure,fasting plasma glucose(FPG),glucose loading 2 h plasma glucose(2hPG),and 2 h plasma insulin(2hINS).Vaspin level positively correlated with FPG,2hPG,and 2hINS.Adiponectin level negatively correlated with BMI,WC,FPG,2hPG,and FINS,and positively correlated with HDL-C.A multiple Logistic regression analysis revealed that these three adipocytokines independently correlated with FPG.Conclusions Visfatin and adiponectin levels changed in obese children,associated with abdominal obesity and glucose metabolism.Vaspin might play an important role in regulating glucose metabolism.
