尿外渗及肾周尿瘤的CT诊断

CT Findings of Urinary Extravasation and Urinoma

目的探讨尿外渗、尿瘤的发生机理,分析其CT表现。方法回顾性分析经手术病理证实或临床治疗随访证实的12例尿外渗或尿瘤病例的影像资料,其中5例平扫后行增强扫描。结果 12例患者CT平扫显示均有患肾体积增大,肾盂和输尿管中～重度扩张积水;5例肾周、肾门、肾包膜下或输尿管尿周围絮状边缘模糊的密度增高影;9例肾周间隙囊状水样密度影;8例有腹膜炎、腹膜增厚和肾周筋膜增厚改变。结论外伤及尿路梗阻是尿外渗、尿瘤形成的必要条件。CT对尿外渗及尿瘤的诊断有很高的价值。

objective To investigate mechanism and CT findings of the urinary extravasation and urinoma.Material and methods The CT findings of 12 cases with urinary extravasation and urinomaw were analyzed retrospectively,which were final diagnosed pathologically or follow up.5 cases underwent CT Plain scan and contrast enhanced scan.Results 12 cases,unenhanced CT showed increase of kidney volume,mild or severe hydronephrosis and hydroureter.The effusion was found all around kidney,ureter and renal hilum in 5 cases,9 cases showed Perinephric effusion like capsular,8 cases showed peritonitis,the increase of peritoneum and perirenal fascia.Conclusion Blunt renal injury and the hypertension of upper urinary tract are the essential condition of urinary extravasation and urinoma.CT has great diagnostic value in urinary extravasation and urinoma.
