卒中单元模式下对脑卒中患者平均住院日和死亡率的影响

Influence of stroke unit on the length of staying hospital and mortality for patients with stroke

目的:探讨卒中单元模式下对脑卒中患者平均住院日及死亡率的影响。方法:选择2008年1月～2009年3月年我院收治卒中单元治疗的221例脑卒中患者为A组,同期接受常规治疗的261例脑卒中患者为B组,对他们的平均住院时间和住院死亡率进行比较。结果:A组患者平均住院日(15±9.2)d,B组患者平均住院日(19±8.7)d,两者比较有统计学意义(P<0.01)。两组患者住院日中位数为7d,以7d一分为二,A组患者住院日超过7d的占40.40%,B组占54.80%,两者比较亦有统计学意义(P<0.01)。A组死亡率为5.43%,B组死亡率为14.93%,两组比较,P<0.01。结论:脑卒中单元的建立明显缩短了患者平均住院日和死亡率,大大地改善了患者预后,提高了患者生存质量。

Objective：Compared to general neurology ward, to investigate the influence of a stroke unit on length of stay and in - hospital ease fatality, Methods ： 221 of stroke patients admitted to stroke unit of neurology department were assigned into group A between January 2008 and March 2009. During the corresponding period, 261 of stroke patients admitted to general neurology, ward were assigned into group B. The length of hospital stay and in - hospital case fatality of two groups were compared. Results：Average length of stay for patients of group A was （ 15±9.2） days, （ 19 ± 8.7 ） days for patients of group B, with a significant difference （ P 〈 0.01 ）. The length of stay was dichotomized after being centered to 7 days, The proportion of patients with length of stay 〉 7 days on general neurology wards was 54.80% Vs 40.40% on the stroke unit （ P 〈 0.01 ）. The mortality of group A was 5.43% , which of group B was 14.93% , with a significant difference（ P 〈 0.01 ）. Conclusion ： We observed a reduced length of stay and reduced in - hospital case - fatality in a stroke unit compared to general neurology wards, which improve the prognosis and cost benefit of patient and promote the business benefits of hospital.
