尿单核细胞趋化蛋白1和尿视黄醇结合蛋白与糖尿病肾病的关系 被引量:11

Relationship of urinary monocyte chemoattractant protein-1 and urinary retinol binding protein with diabetic nephropathy

摘要 目的探讨尿单核细胞趋化蛋白1(MCP-1)、尿视黄醇结合蛋白(RBP)在糖尿病肾病(DN)中的作用及关系。方法对90例糖尿病患者按尿白蛋白排泄率(UAER)分为正常白蛋白尿(NA)组、微量白蛋白尿(MA)组、大量白蛋白尿(LA)组共3组,分别测定尿MCP-1与尿RBP,并与30名健康对照(NC)组比较。结果 NC、NA、MA、LA组间的尿MCP-1及尿RBP水平逐渐升高(P<0.05或P<0.01);尿MCP-1水平与尿RBP(P<0.01)、UAER(P<0.01)均呈正相关;尿RBP与UAER呈正相关(P<0.01)。结论糖尿病肾病患者随着尿蛋白的升高,尿MCP-1和尿RBP也逐渐升高。 Objective To study the changes of the urinary level of monoeyte chemoattractant protein-1 (MCP1) and retinol binding protein(RBP) and their relationship with diabetic nephropathy. Methods According to urinary albumin excretion rate (UAER), 90 diabetic patients were divided into three subgroups : normo-albuminuria(NA), micro alhuminuria(MA) and clinical alhuminuria (LA). Urinary levels of MCP-1 and RBP were determined respectively,and compared with those of 30 normal controls(NC). Results In type 2 diabetic patients, the levels of urinary MCP-1 and RBP in LA group(476.38 ± 86.32,58.42± 25.33) were significantly higher than those in NC group(79.22±7.30,10.84±7. 17;all P〈0.01)and NA group (147.35±49.62,20. 18±9. 31;all P.〈0.01)and MA group (321.61±65. 18,47.67±18.65;all P 〈0.05). The levels of urinary MCP-1 and RBP were significantly higher in MA than group in NC group(P 〈0.01 )and in NA group(P〈0.01). The levels of urinary MCP-I and RBP in NA group were significantly higher than in NC group(P〈0.01 or P〈0. 05). Urinary MCP-1 correlated positively with urinary RBP (r =0. 837,P〈0.01) and UAER(r= 0. 891 ,P〈0.01). Urinary RBP correlated positively with UAER(r= 0. 903,P〈0.01). Conclusions The urinary MCP-1 and RBP are gradually increased along with UAER increment in patients with diabetic nephropathy.
