基于医疗信息平台的三级双向转诊模式实现

Implementation of Dual Referral between Third-level Hospitals Based on Medical Information Technique

目的:建立基于医疗信息平台的双向转诊模式,实时共享患者信息。方法:将双向转诊系统与医疗机构HIS系统对接,实现患者病历信息的实时传送,并自动将出入院详细记录传送给接收医院。引入省级、市县级及社区三层次不同医疗等级医院,针对患者病情合理分配转诊方向。结果:基于医疗信息平台的三级双向转诊模式,实现了患者病历信息的实时共享。上转病例161例,下转病例95例,居家健康支持367例。结论:采用基于医疗信息平台的三级双向转诊模式,实现了医疗资源的合理分配,提高了高等级医院的诊治效率。提供了较多居家健康支持服务,提高了基层医院的卫生服务水平。

To establish a dual referral between Third-level hospitals based on medical information platform and share patient information. Method Dual referral system was connected to the HIS system to transport patient＇s medica records. Provincial, city, and community medical institutes were included in the project for a rational referral of patients Result Based on medical information platform for the dual referral pattern with Third-level hospitals, real time shar of medical records of patients was achieved. The number of referral to high-grade hospitals was 161. The number ng of referral to low-grade hospitals was 95. And the number of family health service was 367. Conclusion Dual referral based on medical information platform realized the rational distribution of medical resources and improved the efficiency of high-grade hospital. The health service provided by community hospitals was also improved
