2型糖尿病患者脂代谢紊乱与大血管病变的关系 被引量:20

Relationship between lipid disorders and macroangiopathy in patients with type 2 diabetes mellitus

摘要 目的探讨2型糖尿病患者脂代谢紊乱与大血管病变的关系。方法选取50例2型糖尿病患者,按有无大血管病变分为A组及B组。检测空腹血糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)、脂蛋白a(Lp(a))水平,计算ApoA1/ApoB比值。结果 A组FBG、HbA1c、FINS、TC、TG、LDL-C、ApoB、Lp(a)明显高于B组(P均<0.05),HDL-C、ApoA1、ApoA1/ApoB明显低于B组(P均<0.05)。结论 2型糖尿病伴发大血管病变者存在明显脂代谢紊乱。胰岛素抵抗、糖脂代谢紊乱与大血管病变密切相关。积极调控糖尿病患者血糖、血脂水平及改善胰岛素抵抗,对预防及延缓糖尿病大血管病变的发生发展有重要意义。 Objective It is to explore the relationship between lipid disorders and macroangiopathy in the patients diagnosed with type 2 diabetes mellitus(T2DM).Methods 50 cases of patients with T2DM were divided into group A with macroangiopathy and group B without macroangiopathy.Fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),forensically informative nucleotide sequencing(FINS),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesferol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein A1(ApoA1),apolipoprotein B(ApoB),Lipoprotein a in both groups were measured and ApoA1/ ApoB was calculated.Results Levels of FBS,HbA1c,FINS,TC,TG,LDL-C,ApoB,Lp(a) were higher,and levels of HDL-C,ApoA1,ApoA1/ ApoB were lower in group A than that in group B(P all 0.05).Conclusion There was obvious lipid disorder in T2DM patients with macroangiopathy.Insulin resistance and glycolipids disorders may be closely related to macroangiopathy.To control levels of blood glucose and blood fat constructively and to ameliorate insulin resistance may prevent or postpone macroangiopathy progress for T2DM patients.
