冠心病慢性心力衰竭患者血清脂联素水平及芪参益气滴丸对其影响 被引量:10

摘要 目的:探讨芪参益气滴丸对冠心病慢性心衰患者(CHF)血清脂联素(APN)水平的影响。方法:选取我院2009年3月至2010年2月住院的冠心病慢性心衰患者90例,按NYHA分为三组:NYHAⅡ级组(n=30),NYHAⅢ级组(n=30),NYHAⅣ级组(n=30),选择确诊冠心病无心衰的患者30例作为对照组。测定患者血清APN及高敏C反应蛋白(hs-CRP)水平。将其中60例中医辩证为气虚血瘀型的心衰患者进行药物干预,对照组给予常规西医治疗,试验组在西医治疗的基础上给与芪参益气滴丸治疗。治疗1月后,复查血清APN和hs-CRP水平。结果:心衰患者血清APN水平较心功能正常者显著降低,随着心功能恶化,冠心病慢性心衰患者的血清APN水平逐渐减低(P<0.01);试验组升高APN作用优于对照组(P<0.05),试验组有效率高于对照组(P<0.05)。结论:APN能在一定程度上反映冠心病CHF患者的病情程度;芪参益气滴丸有升高血清APN的作用,对改善心功能有显著疗效。 Objective:To investigate the serum adiponectin levels of patients of chronic heart failure with coronary artery disease and the effect of Qishenyiqi dropping pill on serum APN levels. Method:Choose 90 inhospital cases of chronic heart failure with coronary artery disease in 2 nd hospital Tianjin medical University from March, 2009 to February , 2010. According to the criterion of NYHA classes and relative grades, 90 patients who were diagnosised of chronic heart failure with coronary artery disease into three groups:NYHA Ⅱgroup (n=30) , NYHAⅢ group (n=30), NYHA Ⅳ group(n=30). 30 age, BMI and gender matched controls that with coronary artery disease but no HF. Measure serum adiponectin and High sensitivity C-reactive protein level. 60 patients were differentiated by traditional chinese medicine as pattern of qi deficiency and blood stasis, 30 patients with classⅢ, 30 patients with classⅣ, divide these 60 patients into 2 groups, the experiment group (n=30) with conventional therapy and Qishenyiqi Dropping pill, the control group with conventional therapy. Result:Along with the aggravation of heart function, serum adiponectin decrease(P0.01). The experiment group increase serum adiponectin level better than the control group(P0.05). The experiment group's effect was superior than the control group(P0.05). Conclusion:Serum APN can evaluate the level of heart failure. Qishenyiqi dropping pill can increase serum adiponectin level and improve heart function.
