摘要
目的探讨低氧诱导因子-1α(HIF-1α)、增殖指标(Ki67)及微血管密度(MVD)在人弥漫浸润性星形细胞瘤中的表达与临床病理指标及预后的关系。方法用免疫组织化学法检测10例正常人脑组织和76例弥漫浸润性星形细胞瘤中HIF-1α、Ki67和CD34的表达,并以CD34平均阳性血管数作为微血管密度(MVD)。结果 76例弥漫浸润性星形细胞瘤中HIF-1α阳性表达59例(77.6%),Ki67平均核阳性率为8.22%,平均MVD为(43.7±11.3),与正常对照组比较差异均有统计学意义(P<0.05)。HIF-1α、Ki67的表达率和MVD计数均随弥漫浸润性星形细胞瘤级别的升高而增加,并与患者的预后负相关,各组间差异显著(P<0.01);HIF-1α和K i67的表达与MVD计数呈正相关。结论 HIF-1α、Ki67和MVD计数在弥漫浸润性星形细胞瘤中的表达与级别及预后有关,提示缺氧及微血管的增殖在弥漫浸润性星形细胞瘤的发生和演进中发挥作用。
Objective To analyze the expression feature of hypoxia inducible factor-1α(HIF-1α),cell proliferation index Ki67 and microvessel density(MVD) in diffusely infiltrating astrocytoma,and to explore their relationship with clinicopathologic characteristics and prognosis.Methods The expressions of HIF-1α,Ki67 and CD34 were detected in 76 diffusely infiltrating astrocytomas and 10 normal brain tissues by immunohistochemistry,and MVD was counted by CD34 stained blood vessels.Results The expressions of HIF-1α(77.6%,59/76),average proliferative index Ki67(8.22%) and average MVD(43.7±11.3) in diffusely infiltrating astrocytomas were significantly higher than those in the normal control(P0.05).The expressions of HIF-1α and Ki67 increased with the increasing grade(P0.01) and were associated with recurrence or death(P0.01).MVD was positively correlated with the expressions of HIF-1α and Ki67(r=0.901,P0.05).Conclusion The over expression of HIF-1α,Ki67 and MVD in diffusely infiltrating astrocytomas are closely related with grade and prognosis,which implied that hypoxia,cell proliferation and angiogenesis may play an role in the genesis and progressing of diffuse infiltrating astrocytoma.
