摘要
为预防老年人眼病手术中心血管疾病的发作，选择５８例眼病手术进行动态心电图监测，并分析记录手术中各个步骤时发生的心律失常及ＳＴ－Ｔ变化情况。结果表明，异常动态心电图４６例，占７９．３％，频发室性早搏１６例，占２７．６％，术中牵拉眼肌或压迫眼球时发生窦性心动过缓２１例，占３６．２％，其中１例合并出现Ⅱ度Ⅱ型房室传导阻滞，１例发生窦性静止。局部应用肾上腺素时２例发生室性心动过速，出现缺血性ＳＴ－Ｔ改变３３例，占５６．９％。提示老年人除急症眼病手术外对有严重心血管疾病者，应进行正确有效地治疗，待病情稳定后，再行眼病手术。术前术中用药，如甘露醇、肾上腺素、新福林等，都要考虑到对心血管疾病的影响。术中操作要轻巧，避免眼心反射的发生。
In order to study the prevention of cardiovascular diseases for the aged
during ophthalmic operations,the arrhythmias and STT changes on Holter electrocardiographic
monitoring were recorded and analyzed in 58 cases during ophthalmic operations.The result
showed that the abnormal Holter electrocardiography were found in 46 cases (79.3%);ventricular
premature beats in 16 cases(27.6%).In 21 cases(36.2%),sinus bradycardias were caused by
dragging ophthalmic muscles and pressing eyeball during operations;0type atrioventricular
conduction block was found in one of the patients sinus arrest in one case.Ventricular
tachycardia was found on injecting locally adrenatinum in two cases.Ischemic STT changes
were found in 33 cases (56.9%).Except urgent ophthalmic operation,the aged with serious
cardiovascular diseases would be treated effectively.The operation is performed when they are
good.The affection of the drugs,such as adrenalinum,neoprynum and mannitls on
cardiovascular diseases must be considered during and before operation.The operation is
handy,so that eyeheart reflection is avoided.
