睾丸粘膜相关淋巴组织型淋巴瘤临床病理特征

Primary testicular lymphoma of mucosa associated lymphoid tissue

目的：探讨睾丸粘膜相关组织（ＭＡＬＴ）淋巴瘤的临床病理特征，为临床早期诊断提供依据。方法：对４例睾丸ＭＡＬＴ淋巴瘤作临床病理观察，同时采用Ｓ－Ｐ法或ＡＢＣ法进行ＬＣＡ、Ｌ２６、ＵＣＨＬ１、Ｂｃｌ－２免疫组化标记。结果：患者临床症状为睾丸不同程度肿大。组织学图象与胃肠道ＭＡＬＴ淋巴瘤相似，１例转化为高度恶性ＭＡＬＴ淋巴瘤。免疫组化结果显示瘤细胞表达ＬＣＡ、Ｌ２６和Ｂｃｌ－２。结论：老年人出现睾丸弥漫性肿大，首先应考虑睾丸淋巴瘤的可能。该病的确诊依赖病理诊断。

PURPOSE To investigate 4 cases of primary testicular malignant lymphoma of mucosa associated lymphoid tissue (MALT) and analyze their clinical manifestations and pathological features and improve the clinical early diagnosis of testicular MALT. Methods Histopathological method was used to study the morphological characteristics of MALT of testis. The immunohistochemical determination of LCA、L 26 、UCHL 1、Bcl 2 protein of the routine paraffin samples of 4 cases with testicular MALT oma was carried out using S P or ABC method. Results The main presenting symptom was swollen testis without other specific findings. Histologically,they were all diffuse non Hodgkins lymphoma. The histological characteristic of testicular MALT were similar to gastriointestine MALT oma. The results of immunohistochemistry showed that the lymphoma cells were derived from B lymphocyte. LCA, L 26 and Bcl 2 were positive in MALT of testis. Conclusion When a diffuse mass of a testis occurs in a patient with old age, the possibility of lymphoma should be considered first. The definite diagnosis of the disease depends on the pathological examination, and immunohistochemistry plays an important role in differential diagnosis.
