实时三维超声成像诊断胎儿畸形的临床应用研究

摘要 目的探讨实时三维超声在产前诊断胎儿畸形的临床价值。方法对妊娠中晚期的胎儿进行常规二维超声检查、记录,疑似胎儿发育异常的进行3D成像、记录,采集胎儿立体、运动图像,获得形态及行为学影像资料,存入工作站,检查完后自然回放图像与二维图像对比,据此做出诊断。结果发现胎儿畸形51例68处,二维超声诊断与产后诊断相符的有49处(符合率72.1%),三维超声诊断65处(符合率95.6%),二者比较有显著性差异(P<0.05)。结论实时三维超声能够实时直观地显示胎儿各部位解剖结构的三维立体图像,比传统的二维超声提供更丰富的图像信息,同时以它立体成像的优势得到广大受检者及临床医生的认可,是目前产前诊断的可靠技术手段,可在临床推广应用。 Objective To study the value of application of real-time three dimensional ultrasonography on the diagnosis of fetal malformations.Methods After two dimensional ultrasound examination on fetus during the middle and late pregnant periods;abnormal developing fetus are selected to examine and record with real-time three dimensional ultrasonography;Fetal solid images;movement images;morphological and behavior materials are also collected and stored.The comparision of the images between two dimensional ultrasound examination and real-time three dimensional ultrasonography helps diagnose.Results 49 and 65 fetal malformations in 51 cases were found with 2D and 3D and the rate of fully accurate diagnosis was 72.1;and 95.6 %,respectively(P0.05).Conclusion The real-time three dimensional ultrasonography can display the tomographic images at different directions and provide more audio-visual information than conventional two dimensional ultrasonography.It is of solid images superiority and gains admitment of patients and doctors.So it is a dependable technology method in prenatal examination now and it can be spread and applied in clinical practices.
