喉癌螺旋CT与病理侵犯深度的相关性研究

The Correlation of Laryngeal Carcinoma Invasion Presented on Multislice Spiral CT and histopathologic stain

目的通过比较CT及喉癌真实侵犯深度得出二者的数量关系,利用CT检查推算出肿瘤真实侵犯深度,为手术方案、安全缘提供准确参考指标。方法测量28例喉癌患者CT中侵犯深度和喉癌真实侵犯深度(由喉癌病理侵犯深度推算出),计算二者之间关系。结果喉癌局部真实侵犯深度与螺旋CT影像学侵犯深度,有显著差异,P<0.01;二者呈正相关,r=0.653,P<0.01;二者差值的95%可信区间为:1.67-2.29 mm;平均值为:1.98土0.69 mm。结论①CT可较精确反映喉癌真实局部侵犯深度。②喉癌病理和真实侵犯深度与CT侵犯深度三者间有明显正相关性。③喉癌真实侵犯范围=CT测量值+[1.67～2.29]mm;④甲状软骨内、外膜是防止喉癌局部侵犯的天然屏障。

Objective In order to provide some parameters to surgeon before laryngeal carcinoma operation,we studied the correlation of laryngeal carcinoma invasion presented on multislice spiral CT and histopathologic stain. Methods We measured 28 laryngeal cancer patents＇Methods Spiral CT（MSCT） and their cut tissue（immunohistochemical staining with the specific marker CK19）.Their quantitative relation was calculated.Results The correlation of laryngeal carcinoma invasion presented on MSCT and histopathologic stain was different（P〈0.01） and positive correlation（r=0.653,P〈0.01）.Their 95% confidence interval ：1.67-2.29 mm;general average： 1.98±0.69 mm.Conclusion ①MSCT could accurately reflect the real pathological invasion level in laryngeal carcinoma patient.②Laryngeal carcinoma real invasion depth,histopathologic depth and presented on MSCT showed positive correlation.③The real invasion depth of laryngeal carcinoma was equal to data from MSCT add [1.67-2.29] mm.④Thyroid cartilage＇ inner membrane and outer membrane could prevent cancer cell invading.
