下腰椎结核手术并发症分析

Analysis of the Complication Caused by Surgery of Low Lumbar Tuberculosis

目的分析下腰椎结核手术并发症发生情况,提出相应预防措施。方法回顾分析本院进行的下腰椎结核手术220例,统计其病变节段、手术入路、伴随症状、并发症等。赋值并录入SPSS13.0软件,建立标准化数据库,采用相应统计学方法进行处理。结果 220例行下腰椎结核手术患者中,共有48例患者出现10种,55例次术中、术后并发症,包括切口感染,血管损伤,神经损伤,双下肢血栓形成,内固定松动等,发生率为25%,无死亡病例。神经损伤、血管损伤发生率高于其他并发症的发生率,前路手术并发症高于后路入路手术并发症。结论下腰椎结核手术并发症的发生与下腰椎手术解剖部位特殊,病人自身因素及术者的手术技巧等有关。熟悉这一部位解剖,提高手术技巧,术后处理规范是预防此类并发症发生的重要措施。

Objective To analyse the complication caused by surgey of low lumbar tuberculosis and give measures to prevent the complications.Method 220 patients who got low lumbar tuberclusis operation from January 1997 to December 2008 were retrospectively reviewed.The causes and incidence of complication were analysed.Result There were 48 cases having complications with the rate of 25%.The complications include infection of incisional,thrombogenesis vessels and nerve injury,the loose of internal fixation etc,most of which were related to inappropriate surgery and postoperative treatment.Conclusion Being familiar to the anatomy of low lumbar vertebrae and skillful operation combined with appropriate aftertreatment is the key to prevent the complication of low lumbar tuberclusis surgery.
