The Role of CD4^+CD25^+ Regulatory T Cells in the Hepatitis B Patients

摘要 目的探讨CD4+CD25+调节性T细胞在乙型肝炎患者发病机制中的作用。方法利用免疫荧光法检测20例携带者、10例慢性乙型肝炎5、例慢性重症乙型肝炎患者的CD4+CD25+调节性T细胞水平,分析与HBV基因型的关系。结果①重型乙型肝炎组外周血CD4+CD25+调节性T细胞的百分率为(2.52±0.94)%,较慢性乙型肝炎组的(5.25±2.17)%差异有统计学意义(P<0.05),比乙肝病毒携带者组及健康对照组则差异有统计学意义(P均<0.01);②慢性乙型肝炎组外周血CD4+CD25+调节性T细胞的百分率与无症状乙肝病毒携带者组及健康对照组相比,差异具有统计学意义(P<0.05);③乙肝病毒携带者组外周血CD4+CD25+调节性T细胞的百分率为(8.51±2.82)%,与健康对照组的(8.51±2.82)%比较差异无统计学意义(P>0.05)。④HBV基因型C型组外周血CD4+CD25+调节性T细胞的百分率为(6.42±2.36)%,较HBV基因型B型组的(7.64±2.87)%稍为下降,但差异无统计学意义(P>0.05)。结论 CD4+CD25+调节性T细胞能抑制T细胞对乙肝病毒的免疫反应,从而抑制乙型肝炎的发生;CD4+CD25+调节性T细胞的水平与病情轻重有一定的关系。 Objective To explore the possible role of CD4+CD25+Regulatory T cells in the hepatitis B patients.Methods The frequency of CD4+CD25+T cells in the peripheral blood mononuclear cells(PBMCs) from 5 cases of severe hepatitis B patients,10 cases of chronic hepatitis B patients,20 cases of chronic HBV carriers and 8 cases of healthy controls were detected by flow cytometry and restriction fragment length polymorphism(RFLP) analysis assay amplifying S-segment sequences of HBV DNA were used for analyzing the distribution of HBV genotypes.Results A lower proportion of CD4+CD25+Regulatory T Cells in PBMCs was found in severe hepatitis B patients(2.52±0.94)% when compared with chronic hepatitis B patients[(5.25±2.17)%,P<0.05)],chronic HBV carriers[(8.51±2.82)%,P<0.01] and healthy controls[(8.51±2.82)%,P<0.01].there was significantly difference on the frequency of CD4+CD25+ Regulatory T Cells in PBMCs between chronic hepatitis B patients and HBV carriers(P<0.05).But no statistically significant differences were found between asymptomatic HBV carriers and healthy controls(P>0.05).Conclusion CD4+CD25+Regulatory T Cells can inhibit the Hepatitis B virus's response to T cells,so as to prevent the development of hepatitis;and there was the relationship between the level of CD4+CD25+Regulatory T Cells and severity of patients' condition.
