加强观察法在急性上消化道出血中的护理应用

The Role of Nursing in Managing Patients with Acute Upper Gastrointestinal Bleed with Strengthened Observation

目的探讨加强观察法在急性上消化道出血护理中的实践和意义。方法根据患者入院的时间,选取因胃溃疡和十二指肠溃疡引起的急性上消化道出血患者78例,随机分为观察组和对照组。观察组采取常规护理+预见性护理+择病情双重观察+患者家属互动参与观察,对照组实施常规护理。应用统计学中的χ2检验,最后评估两组患者的病情发展,休克发生的比例,患者对护理的满意度。结果观察组的休克比例少于对照组,患者满意度显著大于对照组,差异具有统计学意义(P<0.01)。结论加强观察法可为患者赢得宝贵的救治时间,防患于未然,避免了出血性休克的发生。显著提高了患者的护理满意度,增强了护理人员的责任感,对病情的观察既可突出重点,又可相互补充,减少了医疗纠纷。

Objective To investigate the role of the nursing in managing patient with acute upper gastrointestinal bleed with strengthened observation. Methods According to the time of hospital admission of patients,78 cases of patients who had gastric ulcer and duodenal ulcer caused by acute upper gastrointestinal bleeding were selected, and were randomly divided into observation and control groups. General nursing care, anticipatory care, dual observation due to disease and interactive participatory observation of patients＇ family members were adopted in observation group, while general care was adopted in control group. Patients＇ condition progression ,occurrence ratio of shock and level of satisfaction of nursing of the two groups were finally evaluated based on x2 examination in statistics. Results Ratio of shock in observation group was less than that in control group, and level of sat- isfaction in observation group was significantly larger than that in control group. The deviation showed statistical significance（ P 〈 0. O1 ）. Conclusion Strengthening the observation can save valuable treatment time fbr patients, and take preventive measures, therefore, avoid the occurrence of hemorrhagic shock. Besides, this method can significantly improve the satisfaction of nursing and enhance responsibility of the nurse staff. Moreover, the observation of patient was not only focused on the key cases, but also complemented each other, and hence, reduced medical disputes.
