四种不同公式估测胎儿体重准确性的比较 被引量:3

摘要 目的:探讨孕晚期估计胎儿体重最适宜的公式,通过比较四种不同公式对胎儿体重估计的准确性,选择出理想公式作为临床的参考。方法:选择2008年3月至2008年10月分娩于大连医科大学附属第一医院产科的221例无妊娠合并症的孕妇,于分娩前3天由一名有经验的超声医师测量胎儿各径线:双顶径(biparietal diameter,BPD)、头围(head circumference,HC)、腹围(abdominal circumference,AC)和股骨长度(femur length,FL)。分别用四种公式对胎儿体重进行估计,与分娩后出生体重进行比较,分析四种公式的准确性。四种公式分别为,公式1:EFW=1.07×BPD3+0.3×AC2×FL;公式2:Log10EFW=1.326-0.00326×AC×FL+0.0107×HC+0.0438×AC+0.158×FL;公式3:Log10EFW=1.3596+0.0064×HC+0.0424×AC+0.174×FL+0.00061×BPD×AC-0.00386×AC×FL;公式4:EFW=0.23718×AC2×FL+0.03312×HC3。结果:①四种公式估计孕晚期胎儿体重误差百分比均在10%以内。②四种公式在对221例胎儿体重估测与实际体重比较中,公式1预测胎儿体重值与实际体重值比较,无明显差异性,(p>0.05);其余3公式预测胎重值与实际体重比较,均明显小于实际体重,(p<0.05)。③四种公式在对178例正常体重组及41例巨大儿胎儿体重组预测中,其预测体重值与实际体重之间的绝对误差平均值比较分析,公式1明显小于公式2与公式4的绝对误差平均值,公式2、公式4分别与公式1比较(p<0.05);公式3与公式1的绝对误差平均值比较(p>0.05),二者无显著性差异。对于2例低体重儿组因例数较少,无统计学意义,需进一步临床研究。结论:四种公式预测胎重误差均在10%以内,均可以用于临床,其中公式1简单、误差小,更适用于孕晚期胎儿体重的估计。
