获得性非真菌性甲病的组织病理学分析与诊断

Analysis of histopathology of acquired non-fungal onychopathy

目的探讨通过甲单位病理改变对各类获得性非真菌性甲病提供诊断依据。方法 78例无任何皮肤、黏膜损害的甲病患者,通过纵行甲活检手术取得甲标本,进行甲病理学检查。结果根据甲单位病理改变,诊断为甲非特异性湿疹样变24例、甲扁平苔癣22例、甲银屑病20例、连续性肢端皮炎6例、甲母质痣4例和甲下肢端恶性黑色瘤2例。结论甲病理学检查为获得性非真菌性甲病的诊断和治疗提供了重要的依据。纵行甲活检手术安全,不会引起甲部的严重畸形。

Objective To provide evidence on the diagnosis of acquired non-fungal onychopathy by nail unit histopathologic change. Methods We made histopathologic examination by longitudinal nail biopsy operation on 78 cases of onychopathy without any lesion of skin and mucous membrane. Results According to nail unit histopathologic change, there were 24 cases of nail nonspecifie eczematous changes,22 cases of nail lichen planus,20 cases of nail psoriasis,6 cases of acrodermatitis continua,4 cases of nevi of the nail matrix and 2 cases of acral lentiginous melanoma. Conclusion Nail histopathologic examination could provide important evidence on the diagnosis and treatment of acquired non-fungal onyehopathy. Longitudinal hall biopsy operation is safe and will not lead to severe abnormity of nail.
