糖皮质类固醇相关的高眼压症或青光眼——对定义及分类的初步认识

本文对１９例用糖皮质类固醇（激素）后眼压升高的患者进行了全面检查和追踪观察。综合分析患者用药前、后特别是停药后的眼压、房角、眼底、视野等资料后，分为以下４组：①３例药前各项检查正常，药后眼压升高，停药后观察２～４月，眼压尚需用降眼压药维持；但眼底、视野正常。②９例药前各项检查正常，药后眼压短期内升高，停药后各项检查正常。③５例药前或药后追查证实药前已有眼压、房角等异常；药后眼压显著升高，停药后不能恢复正常。④２例药前各项检查正常，药后眼压升高并已造成视盘、视野损害。根据以上结果，对糖皮质类固醇相关的高眼压症或青光眼之定义及分类提出了初步认识。

cases of ocular hypertension or glaucoma found after treatment with glucocorticoid were systematocally examined and followed up.The data of intraocular pressure,anterior chamber angle,fundus and visual field before,during and after applying glucoticiod treatment were analyzed synthetically.The patients were divided into 4 groups:(1)3 of them with normal IOP pretreatment got a raised IOp that not returned to normal level without antiglaucoma drugs within 2 4 months after stopping the glucoticiod treatment,but their fundus and visual field remained normal.(2)9 cases with a normal IOP pretreatment got a raised IOP after using glucoticiod,but their IOPs became normal shortly after stopping the treatment and their fundus and visual field also remained normal.(3)5 cases got a raised IOP that could not return to normal after stopping glucoticiod treatment,but they all were proved of abnormality of IOP or the anteior chamber angle before the use of glucocorticoid.(4)2 cases with a normal IOP pretreatment got a raised IOP,enlarged cup and visual field damage of glaucoma after using glucoticiod.A primary discussion was made about the definition and classification of glucoticiod correlated ocular hypertension or glaucoma according to our clinical trail.
