肘管指数在肘管综合征治疗中的临床意义

Clinical Significance of Cubital Tunnel Index in the Treatment of Cubitai Tunnel Syndrome

目的：探讨肘管指数作为肘管综合征术前、术后评估指标的临床意义.方法：提出肘管指数的概念,即通过Hueter线（肱骨内、外髁的连线）横截面向前旋转30°行CT平扫,在CT片上测出该截面下肘管的深度和宽度,计算深度/宽度的比值,该比值即为肘管指数.对102名正常成人（志愿者）进行测量,得出的均值设为正常值,与16例肘管综合症患者术前、术后所测出的肘管指数进行比对,并据此作出病因分析及术前评估.结果：正常成人过Hueter线横截面向前旋转30°CT平扫,测得肘管的深度（3.59±0.886）mm、宽度（13.271±2.409）mm,肘管指数为0.273±0.055.16例肘管综合症患者术前肘管指数均增大,其肘管深度为4.294±0.632、宽度为9.26±2.29,肘管指数为0.485±0.098 术后肘管指数均数为0.536±0.047,术前、术后比较差异有统计学意义（P＜0.01）.结论：肘管指数变大,说明骨质增生、尺神经沟变窄,如压迫尺神经则可导致肘管综合征.肘管指数可作为肘管综合征的术前、术后评估指标.

Objective：To investigate the clinical significance of cubital tunnel index in the preoperative and postoperative evaluation for the treatment of cubital tunnel syndrome. Methods ： The concept of cubital tunnel index was proposed： to measure the section depth and width of the cubital tunnel through Hueter line （ Hueter line through the humerus, the internal and external condyle of the connection） section through 30~ rota- tion for the front row CT scan, the depth/width ratio was caculated. 102 normal adults （Volunteer） , combined with 17 cases of cubital tunnel syndrome were measured by comparing cubital tunnel index out, the cause of cu- bital tunnel syndrome was analyzed and preoperative evaluation was made. Results：Normal adults in cross Hueter line section through 300 rotation for the front row CT scan, the depth of cubital tunnel was （3.59± 0.886） mm, the width was （13. 271 ±2. 409）mm, the cubital tunnel index was 0. 273 ± 0. 055. The cubital tunnel index of 16 cases with cubital tunnel syndrome had increased, the depth was （4. 294± 0. 632） ram, the width was （9. 260 ±2. 290） mm, the cubital tunnel index was 0.485 ± 0.098 and the preoperation cubital tunnel index was 0. 536 - 0. 047. Pre - and postoperative differenes had statistical significance（ P 〈 0.01 ）. Conclusions：Larger cubital tunnel index indicates hyperostosis, and the ulnar nerve sulcus becomes narrower. Cu- bital tunnel index can be the preoperative and postoperative assessment of indicators for cubital tunnel syndrome.
