43例胸腰椎爆裂骨折经骨折椎椎弓根固定治疗临床疗效分析 被引量:6

Treatment of thoraco lumbar burst fractures with fixation through the pedicle of fractured vertebra

摘要 [目的]分析和探讨经骨折椎椎弓根固定治疗胸腰椎爆裂性骨折的可行性及临床疗效。[方法]回顾性分析本科自2001年12月~2006年6月收治的71例单椎体胸腰椎爆裂性骨折患者的临床资料,其中单纯行跨骨折椎的4钉固定术(A组)28例,包括骨折椎椎弓根固定的6钉固定术(B组)43例。随访13~34个月(平均27个月),分别从病人骨折椎前缘高度、矢状面Cobb角及额状面Cobb角方面进行评价。[结果]伤椎高度:术前、术后比较U>2.58,P<0.01,表明两组间比较差异有高度统计学意义;额状面Cobb角、矢状面Cobb角:术前、术后比较t>2.807,P<0.01,表明两组间比较差异有高度统计学意义。[结论]经骨折椎椎弓根固定治疗胸腰椎爆裂性骨折是切实可行及有效的方法,值得临床探讨应用。 [Objective]To analyze and investigate the feasibility and clinical therapeutic efficacy of reduction and fixation through the pedicle of fractured vertebra in treatment of thoraco lumbar burst fractures.[Method]Clinical data of 71 cases of one level thoraco lumbar burst fracture in our department from December 2001 to June 2006 were retrospectively analyzed.All the cases were followed up for 13 to 34 months(27 months on average),and two groups were comparative analyzed in terms of fractured vertebra height and the Cobb angle.[Result]While the fractured vertebral height and the Cobb angle of two groups had significant difference immediately(P0.01).[Conclusion]Reduction and fixation through the pedicle of fractured vertebra is an effective therapeutic option to treat the thoraco lumbar burst fractures and deserves clinical study and application.
